Phlebotomy Checklist

To enroll, bring all of these required items with you. No early or partial enrollments or “holds” accepted. Enrollment Must be in Person at the College.

Two (2) forms of identification, including a photo ID and SSN.

Report of TB skin test done within the last year (or TB chest x-ray.)

Documentation of Immunizations or titer for immunity.

  • MMR (2), Chicken Pox/Varicella (2), Hepatitis B (3), Tetanus (1 within last 10 years).
